Ordering Instructions:  Return COMPLETED order form and FULL payment in a sealed envelope to Kara Baker or Front Office.  Shirts will be sold by order ONLY. No extras will be available.  Please make checks out to FMS PTA.  Remember thata too big is better than too small when ordering sizes.
